Alzheimer’s Disease
Alzheimer’s disease is the most common type of dementia. It is a progressive disease beginning with mild memory loss and possibly leading to loss of the ability to carry on a conversation and respond to the environment. (Centers for Disease Control and Prevention)
Mild Cognitive Impairment (MCI)
MCI is a stage where memory problems are worse than those that come with normal aging but are not as severe as Alzheimer’s disease (National Institute on Aging).
HIPAA Authorization
HIPAA stands for the Health Insurance Portability and Accountability Act of 1996. Specifically, the privacy regulation (HIPAA) permits the research investigators outlined in your informed consent document to use and disclose health information about you for the research study identified in the informed consent which has been approved by the Western Institutional Review Board. If you agree to the terms of the Informed Consent for the Minds2021 Study, you are authorizing SteadyMD and Merck & Co., Inc. to release your Protected Health Information (“PHI”) as specified in the HIPAA Authorization to Evidation Health and Merck & Co., Inc. for the purposes of this research. Once it is disclosed to Merck & Co., Inc., and Evidation Health, your PHI will no longer be protected by the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) and may be re-disclosed pursuant to the terms of the signed Informed Consent.
You can withdraw your consent at any time if you change your mind. Beginning on the date you withdraw your consent, no new protected health information will be used for research. However, researchers may continue to use the protected health information that was provided before you withdrew your consent. If you sign this form and enter the research study, but later change your mind and withdraw your consent, you will be removed from the research study at that time.
To withdraw your consent and be removed from the study, login to your Study Dashboard or go to the Minds2021 app. Click on Settings. Once in your Account Settings select Withdraw From Study. You can also email us to process the withdrawal on your behalf.
Informed Consent
The informed consent form provides a short summary of this research study. It describes the key information that we believe most people need to decide whether to take part in this research. This document provides all of the important details that you need to know.
The informed consent document was presented to you at the beginning of the study. A signed copy was emailed to you after you electronically signed the document. You can download a signed copy from your Study Dashboard, as well.
Minds2021 app
The Minds2021 mobile application, created by Evidation Health, will allow you to share your permissioned data needed for the study. You can use this app to complete enrollment for the study, connect additional apps and data for the study, manage your Achievement Studies account, and keep track of your past and upcoming study activities and progress. You can also complete certain study tasks in the app, like the typing tasks and weekly surveys, or check your compensation, and contact support.
For iOS or iPhone users, location access must be set to Always to participate in the Minds 2021 Study. For Android users, location permissions must be set to Allow all the time to participate in the Minds 2021 Study.
Study Dashboard
Your Study Dashboard allows you to connect additional apps and data for the study, manage your Achievement Studies account, and keep track of your past and upcoming study activities and progress. You can also check your compensation, download a copy of your signed informed consent, and contact support. The Study Dashboard can be accessed by logging in on your computer or through the Minds2021 mobile app on your smartphone.
